Terms of Service

1. Company identity

CarebudgetAI is operated by Balance & Wellness L.L.C., a Minnesota limited liability company based in Burnsville, MN. In these Terms, "we," "us," and "our" refer to Balance & Wellness L.L.C. "You" and "your" refer to the individual using CarebudgetAI.

2. What the service is

CarebudgetAI is an informational tool that compares the information you provide against publicly available program eligibility criteria for Medicaid waivers, self-directed care programs, and related benefit pathways. Based on that comparison, we generate a report that highlights programs that may match your situation and suggests next steps you can take to verify eligibility with the appropriate agency. Reports include content generated by artificial intelligence (AI). AI-generated content is produced based on the information you provide and publicly available program data. AI systems can produce errors, omit relevant details, or reflect outdated information. You should treat all AI-generated content as a starting point for further research, not as a definitive or authoritative statement.

3. What the service is NOT

CarebudgetAI does not provide and should not be relied upon as legal advice, medical advice, insurance advice, tax advice, financial planning, or official eligibility determinations. We are not affiliated with, endorsed by, or acting on behalf of any federal, state, or local government agency. We are not a government agency. No output from CarebudgetAI constitutes an application for benefits, a guarantee of eligibility, or an official determination of any kind. You must confirm all program details, eligibility requirements, and enrollment processes directly with the relevant state or county agency or a qualified professional.

4. No guarantees

CarebudgetAI does not guarantee eligibility, enrollment, approval, benefit amounts, waitlist timing, payment amounts, or any other outcome. Program rules, funding, waitlists, and county-level administration change frequently and without notice. The information in your report reflects the data available at the time it was generated and may not reflect current conditions. Any dollar amounts, budget ranges, or pay rates shown in your report are based on published program data and are not a promise of what you will receive.

5. User responsibility

You are solely responsible for verifying all information in your report with the relevant state, county, or federal agency and with qualified professionals before taking any action. CarebudgetAI provides informational starting points, not verified conclusions. You should not make financial, legal, medical, or care decisions based solely on the contents of a CarebudgetAI report without independent verification.

6. User representations

By using CarebudgetAI, you certify that:

• •You are at least 18 years old or are using the service with the consent of a parent or legal guardian.
• •The information you provide in your assessment is truthful and accurate to the best of your knowledge.
• •You understand that the quality and relevance of your report depends on the accuracy of the information you provide.
• •You will not use the service for any unlawful purpose or in a way that could harm, disable, or impair the service.
• •You will not submit information about a third party without their knowledge and consent.

7. Prohibited content

Do not submit any of the following through CarebudgetAI:

• •Social Security numbers (SSNs)
• •Medicare or Medicaid member IDs
• •Bank account numbers, credit card numbers, or other financial account details
• •Full medical records, diagnosis codes, or protected health information beyond what the assessment questions ask

8. Payment terms

The eligibility check (assessment) is free. If programs are matched, you may purchase a full report for a one-time payment of $199 processed through Stripe. There is no subscription and no recurring charges. The report is generated and delivered immediately after payment confirmation. If the eligibility check finds no matching programs, you will not be charged.

9. Refund policy

If you are unsatisfied with your report for any reason, email support@carebudget.org within 30 days of purchase to request a full refund. Refunds are processed within 10 business days. If your report fails to generate or is not delivered due to a technical error, contact support@carebudget.org and we will either re-deliver the report or issue a full refund.

10. Limitation of liability

To the fullest extent permitted by applicable law, Balance & Wellness L.L.C., its owners, officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages — including but not limited to loss of benefits, loss of income, emotional distress, or costs of procuring substitute services — arising out of or related to your use of or inability to use CarebudgetAI, even if we have been advised of the possibility of such damages. Our total aggregate liability for any claim arising from or related to the service shall not exceed the amount you paid us in the 12 months preceding the claim. Some jurisdictions do not allow certain limitations of liability, so some of these limitations may not apply to you.

11. Indemnification

You agree to indemnify, defend, and hold harmless Balance & Wellness L.L.C. and its owners, officers, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or related to: (a) your use of CarebudgetAI; (b) your violation of these Terms; (c) your violation of any third party's rights; or (d) any information you provide through the service.

12. Dispute resolution

These Terms are governed by the laws of the State of Minnesota, without regard to conflict-of-law principles. Any dispute arising from or relating to these Terms or the service shall first be addressed through good-faith negotiation by contacting support@carebudget.org. If the dispute is not resolved within 30 days, either party may pursue resolution in the state or federal courts located in Dakota County, Minnesota, and you consent to personal jurisdiction in those courts.

13. Changes to terms

We may update these Terms from time to time by posting a revised version with a new "Last updated" date. We will not separately notify you of changes. Your continued use of CarebudgetAI after the updated Terms are posted constitutes your acceptance of the updated Terms. We encourage you to review this page periodically.
